    I’m required by House Bill 4224 (89th Regular Session) and The No Surprises Act to inform you of your rights as a consumer. See below:

  • Request Records

    As a client, you may request your health records from me at any time by emailing me at gpetronellatherapist@gmail.com.

  • Contact the BHEC

    My Texas license is regulated by the Texas State Board of Social Worker Examiners, which is housed under the Texas Behavioral Health Executive Council (BHEC). If you would like to contact the BHEC for any reason, you may do so at the BHEC’s Contact Us page.

  • File a Complaint

    You may file a consumer complaint with the Office of Attorney General’s Consumer Protection wepbage.

  • Request Good Faith Estimate

    You have the right to a Good Faith Estimate (GFE). A GFE shows the cost of services that are reasonably expected for your mental health care needs over the course of a year. The estimate is based on information known at the time the estimate is created. The GFE is not a contract and services can be discontinued at any time. The GFE does not include any unknown or unexpected costs that may arise during treatment. You may require more sessions depending on your progress. Any changes to the treatment plan will be discussed with the therapist as needed. If you are a current client and would like to request a GFE, please email me at gpetronellatherapist@gmail.com. Any changes to the cost of therapy sessions will be discussed in advance.
